Terms and conditions of suspending your membership

Members of USNW FAC are eligible for a maximum of 12 weeks per calendar year at no charge

More than 12 weeks required due to medical reasons must be supported by a medical certificate

Requests to suspend must be given with a minimum of 3 days-notice from start date of suspension

Should you wish to extend your hold further than what is requested in this form, you must fill another suspension request form – Otherwise payments will resume as per this request.

3 Month upfront memberships are not entitled to suspend membership

Suspensions are not available for Learn To Swim members – Make up sessions can be organised. Email swimschool@unswfac.com.au for absentee and make up session requests.

In accordance with the guidelines set in the Terms and Conditions of my agreement, I would like to request that my membership be placed on hold for the period set out below:
